More than 16,000 patients of the Dunes Family Health Care clinic in Oregon could be affected by a recent breach of protected health information, Health Data Management reports.
Details of Breach
On March 11, a back-up hard drive that the clinic sent to a contract organization for storage was discovered missing. The hard drive was stored in a locked, fire-protected building, according to a notice on the clinic's website.
The clinic noted that some of the files on the hard drive contained patients' Social Security numbers and other personal information. Other files did not include Social Security numbers but included:
- Names;
- Birth dates;
- Addresses; or
- Clinical information (Goedert, Health Data Management, 5/13).
